Identifying Vaginal Pain

Tips from Alessandra Grazziotin, MD

February, 2004

Describing painful sex can be a challenge. Many women feel self-conscious raising this topic with their clinicians. As a result, their concerns go unaddressed, and over time, problems that could be easily cured, grow worse. Some gynecologists may feel awkward entering the emotionally charged arena of sexuality. For all of these reasons, it helps to have a template for discussion. A good place to begin: this list prepared by medical sexologist, Alessandra Graziottin.

  • When did you first notice that sex was becoming painful?
  • Do you feel pain before, during or after intercourse?
  • How intense is it?
  • Where does it hurt? At the entrance, in the middle of the vagina or deep inside?
  • Do you feel pain every time you have sex, or just when you assume certain positions or are in certain sexual situations?
  • If you avoid penetration, are you still pleasured and satisfied by your sexual experience?
  • Is your vagina dry or irritated during sex? Is it painful to urinate 24-72 hours later?
  • How strong is your sex drive? Are you easily aroused? Have you noticed any changes in your level of desire?
  • Are you incontinent? Do you worry about leaking during sex?
  • Do you have another physical condition that causes anxiety or pain?
  • What medications are you taking? Are you on antidepressants SSRIs such as Prozac, Paxil, Zoloft, Celexa, Lexapro or Effexor)?
  • Have you been depressed, stressed at work or by family responsibilities?
  • Are you unhappy with your current relationship?
  • What is the level of your partner's sexual drive? Does he have difficulty with arousal or sexual function?
